How to Get Columbus Death Certificates
You have several ways to get a death certificate in Columbus. In person is the fastest. Walk into the Foxpointe Drive office, fill out a form, show your ID, and pay the $15 fee. You can have a certified copy in hand the same day.
Mail requests are an option too. Send a letter to the Bartholomew County Health Department with the full name of the deceased, date of death, your name and address, phone number, and a check or money order for $15. Make the payment out to the health department. Allow two to three weeks for processing and shipping. Do not mail cash.
Online ordering goes through VitalChek. You pay the base fee plus a service charge for using a credit card. VitalChek sends the order to the county for fulfillment. The total cost is more than an in-person request, but you can place the order at any time from anywhere. Phone orders through VitalChek are also available.
The Indiana Department of Health is another route. Print Form 49606 and mail it to the state. The state keeps Indiana death records from 1900 forward. This works well when you need records from more than one county.
